UnitedHealth Group’s 2024 financial performance underscores its robust market positioning, with the company reporting $400.3 billion in revenue—a 6% year-over-year increase—and $32.3 billion in full-year operating earnings. CEO Andrew Witty credited much of this success to the pharmacy benefit manager (PBM) OptumRx.

OptumRx Leads the Charge

During the company’s Q4 earnings call, Witty highlighted the pivotal role of PBMs in controlling drug costs, emphasizing OptumRx’s contribution to the company’s savings initiatives.

“Pharmacy benefit managers play a vital role in keeping those prices down,” Witty stated. “OptumRx alone delivers tens of billions of dollars in savings annually.”

Witty also announced a commitment to pass through 100% of drug rebates to consumers by 2028, an increase from the current 98% rate. He encouraged other companies to follow suit to promote transparency in drug pricing, adding, “This will help make clear who is really responsible for drug prices in this country—the drug companies themselves.”

Financial Highlights

  • Optum’s Performance:
Optum’s 2024 revenues reached $253 billion, a 12% year-over-year increase. OptumRx and Optum Health emerged as the primary drivers, with OptumRx revenues rising by 15% due to growth in both new and existing client relationships.
    • Adjusted scripts increased to 1.62 billion from 1.54 billion in 2023.
    • Optum Health revenues grew to $105.4 billion, fueled by value-based care arrangements.
  • UnitedHealthcare’s Growth:
UnitedHealthcare, UHG’s healthcare benefits subsidiary, saw revenues of $298.2 billion (up 6% year-over-year) and operating earnings of $15.6 billion. The domestic commercial benefits market added 2.4 million consumers, and seniors and individuals with complex needs rose to 9.4 million.
  • Challenges:
Despite the gains, UnitedHealth faced obstacles in 2024:
    • Medicare Rate Cuts: These created a $6 billion gap in revenue.
    • Cybersecurity Incident: The Change Healthcare cyberattack incurred an estimated $1 billion loss.
    • Inflation Reduction Act: Accelerated adoption of high-cost medicines added pressure.

The company’s medical care ratio increased to 85.5% in 2024 from 83.2% in 2023, reflecting rising costs attributed to Medicare funding cuts and member mix changes.

Broader Implications

UnitedHealth’s CFO, John Rex, acknowledged these challenges but pointed to the company’s cost management strategies, which helped reduce its operating cost ratio to 13.2%, down from 14.7% in 2023.

The earnings call also marked a somber moment as it was the first following the tragic loss of UnitedHealthcare CEO Brian Thompson. Witty expressed gratitude for the outpouring of support in the wake of Thompson’s death.

Moving Forward

With significant contributions from OptumRx and a clear focus on transparency and cost control, UnitedHealth Group is positioning itself as a leader in addressing systemic challenges in healthcare pricing and delivery.
